Job Summary
MNC JOBS INFO is seeking a compassionate and dedicated Remote Social Worker - Case Manager Assistant to support our mission in delivering high-quality services to individuals and families in need. As a Case Manager Assistant, you will work closely with licensed social workers and other professionals to coordinate care, monitor client progress, and provide administrative and practical support to enhance client outcomes. Key Responsibilities

Assist licensed social workers in managing client caseloads and coordinating care plans.

Conduct initial client intake assessments and gather relevant information for case files.

Maintain accurate and up-to-date client records, reports, and documentation.

Schedule and coordinate appointments, follow-ups, and referrals to support services.

Provide ongoing support and communication to clients, families, and community partners.

Monitor client progress, track outcomes, and report findings to supervising social workers.

Support grant reporting and program evaluation by collecting and organizing data.

Assist with crisis intervention and provide resources in urgent situations, under supervision.
Required Skills and Qualifications

Bachelor’s degree in Social Work (BSW), Psychology, Human Services, or a related field.

Strong organizational and time-management skills.

Excellent written and verbal communication skills.

Empathy, active listening, and a client-centered approach.

Basic knowledge of case management software and digital documentation tools.

Ability to work independently and collaboratively in a remote environment.
Experience

Minimum of 1–2 years of experience in social services, case management, or a related field.

Previous experience in non-profit organizations or community outreach programs is preferred.

Exposure to working with diverse populations, including vulnerable or at-risk clients.
Working Hours

This is a full-time remote position.

Standard working hours are Monday to Friday, 9:00 AM – 5:00 PM (flexible hours may be accommodated).

Occasional evening or weekend work may be required depending on client needs.
