Job Summary
InfoTech Solutions is seeking a dedicated and compassionate Remote Nurse Case Manager with a focus on behavioral health. Key Responsibilities
- Conduct thorough assessments of clients’ behavioral health needs, including medical, psychological, and social factors.

- Coordinate care across multiple providers and service settings, ensuring seamless transitions and continuity of care.
- Educate clients and families on treatment plans, behavioral health strategies, and available community resources.
- Monitor progress, identify barriers to care, and adjust plans as necessary to achieve optimal outcomes.
- Maintain detailed and accurate electronic health records, case notes, and reporting documentation.
- Serve as a patient advocate, ensuring access to quality behavioral health services and adherence to best practices.
- Participate in team meetings, case reviews, and ongoing professional development initiatives.
Required Skills and Qualifications
- Current and active Registered Nurse (RN) license in the state of practice.
- Proven knowledge of behavioral health conditions, treatment modalities, and case management principles.
- Strong clinical assessment and care planning skills.
- Excellent communication and interpersonal skills, with the ability to build trust with clients and families.
- Proficient in electronic health records (EHR) systems and telehealth platforms.
- Strong organizational and time management abilities to handle multiple cases effectively.
- Ability to work independently in a remote environment while maintaining accountability to a multidisciplinary team.
Experience
- Minimum of 3–5 years of nursing experience, preferably in behavioral health, mental health, or case management.
- Experience in remote or telehealth care coordination is highly desirable.
- Previous exposure to behavioral health programs, care transitions, or utilization management is a plus.
Working Hours
- Full-time, remote position with flexible scheduling options to accommodate client needs and time zones.
- Occasional availability for virtual meetings outside standard business hours may be required.
