Job Summary

InfoTech Solutions is seeking a compassionate and results-driven Behavioral Health Care Manager – Remote Support to join our growing healthcare services team. This role is designed for a licensed behavioral health professional who is passionate about improving patient outcomes through care coordination, clinical support, and digital health solutions.

As a Behavioral Health Care Manager, you will work remotely to support individuals with mental and behavioral health needs by providing assessments, care planning, case management, and continuous follow-up. You will collaborate closely with clinicians, healthcare providers, and multidisciplinary teams to ensure patients receive high-quality, timely, and patient-centered care in a virtual environment.

Key Responsibilities

  • Conduct comprehensive behavioral health assessments using telehealth tools.

  • Develop, implement, and monitor individualized care plans based on clinical needs.

  • Provide case management and care coordination for patients with mental health and substance use conditions.

  • Serve as the primary point of contact for patients, families, and healthcare providers.

  • Facilitate referrals to psychiatrists, therapists, social workers, and community resources.

  • Monitor patient progress and adjust care plans accordingly.

  • Maintain accurate and compliant clinical documentation in electronic health records (EHR).

  • Support crisis intervention and escalation protocols when required.

  • Educate patients on treatment plans, medication adherence, and self-management strategies.

  • Participate in interdisciplinary team meetings and quality improvement initiatives.

Required Skills and Qualifications

  • Bachelor’s or Master’s degree in Social Work, Psychology, Nursing, Counseling, or a related healthcare field.

  • Active professional license (LCSW, LMHC, LPC, RN, or equivalent).

  • Strong understanding of behavioral health principles and evidence-based practices.

  • Excellent verbal and written communication skills.

  • Proficiency in telehealth platforms and EHR systems.

  • Ability to work independently in a remote environment.

  • Strong organizational, problem-solving, and analytical skills.

  • High level of empathy, professionalism, and patient-centered mindset.

Experience

  • Minimum of 2–5 years of experience in behavioral health, mental health case management, or clinical care.

  • Prior experience in telehealth or remote healthcare services is highly preferred.

  • Experience working with diverse populations and complex behavioral health cases.

Working Hours

  • Full-time, remote position.

  • Flexible scheduling within standard business hours (Monday to Friday).

  • Occasional evening or weekend availability may be required depending on patient needs.

Knowledge, Skills, and Abilities

  • In-depth knowledge of mental health disorders, treatment modalities, and care pathways.

  • Ability to manage multiple patient cases effectively.

  • Strong clinical judgment and decision-making skills.

  • Cultural competence and sensitivity to diverse patient backgrounds.

  • Ability to maintain confidentiality and comply with HIPAA and regulatory standards.

  • Skilled in motivational interviewing and patient engagement techniques.
