Job Summary:

WellBeing Foundation is a leading non-profit organization dedicated to improving community health and well-being through education, advocacy, and hands-on support. We are seeking a passionate and committed Community Health Worker (CHW) to join our team. The CHW will serve as a bridge between the organization and local communities, delivering health education, supporting health initiatives, and empowering individuals and families to achieve better health outcomes. This role combines field engagement, community outreach, and data-driven reporting to drive meaningful change at the grassroots level.

Key Responsibilities:

  • Conduct community outreach to identify health needs and provide education on preventive care, nutrition, hygiene, and disease management.

  • Serve as a liaison between the community and healthcare providers, ensuring access to essential services.

  • Organize and facilitate health workshops, awareness campaigns, and community events.

  • Support enrollment and navigation of healthcare programs, including vaccination drives and screenings.

  • Collect and maintain accurate data on community health indicators, attendance, and program outcomes.

  • Advocate for community needs to inform program planning and organizational strategy.

  • Collaborate with local health authorities, NGOs, and volunteers to maximize impact.

  • Provide support for crisis intervention and referral to healthcare professionals when necessary.

Required Skills and Qualifications:

  • High school diploma or equivalent (Associate’s or Bachelor’s degree in Public Health, Social Work, or related field preferred).

  • Strong understanding of community health principles and preventive care.

  • Excellent communication, interpersonal, and presentation skills.

  • Ability to build trust and maintain positive relationships with diverse populations.

  • Proficient in basic computer applications and data entry for reporting purposes.

  • Bilingual or multilingual skills are a plus (depending on community served).

Experience:

  • Minimum of 1–2 years of experience in community health, public health programs, social work, or related outreach roles.

  • Prior experience in non-profit organizations, NGOs, or healthcare settings is highly desirable.

Working Hours:

  • Full-time position, typically 40 hours per week.

  • Flexible schedule may include evenings or weekends to attend community events or health drives.

  • Occasional travel to field locations or partner organizations may be required.

Knowledge, Skills, and Abilities:

  • Deep knowledge of local community health challenges and resources.

  • Ability to conduct health education sessions and workshops effectively.

  • Strong organizational and time-management skills.

  • Empathy, patience, and cultural sensitivity when interacting with diverse populations.

  • Ability to work independently as well as part of a collaborative team.

  • Problem-solving skills to address community health barriers proactively.
