Job Summary
InfoTech Solutions is seeking a compassionate and detail-oriented Remote Nurse Case Manager specializing in Behavioral Health to join our growing healthcare support team. In this fully remote role, you will coordinate and manage care for members with behavioral health needs, ensuring they receive appropriate, timely, and cost-effective services. Key Responsibilities

Conduct comprehensive behavioral health assessments and develop individualized care plans.

Coordinate care across providers, facilities, and community resources to support member needs.

Monitor patient progress and adjust care plans based on clinical findings and outcomes.

Provide telephonic and virtual case management support, education, and counseling.

Identify high-risk members and implement appropriate intervention strategies.

Ensure compliance with regulatory, accreditation, and company guidelines.

Maintain accurate and timely documentation in electronic health record systems.

Collaborate with interdisciplinary teams including physicians, therapists, and social workers.

Support utilization management and cost-containment initiatives when appropriate.

Advocate for members to ensure access to quality behavioral health services.
Required Skills and Qualifications

Active and unrestricted Registered Nurse (RN) license.

Bachelor of Science in Nursing (BSN) preferred.

Strong understanding of behavioral health conditions and treatment modalities.

Excellent telephonic communication and interpersonal skills.

Proficiency with electronic medical records (EMR) and case management systems.

Ability to work independently in a fully remote environment.

Knowledge of HIPAA and patient confidentiality requirements.
Experience

Minimum 3–5 years of clinical nursing experience required.

At least 2 years of case management or behavioral health experience preferred.

Prior remote or telehealth experience is an advantage.

Experience working with managed care, Medicaid, or commercial populations preferred.
Working Hours
