Job Summary
InfoTech Solutions is seeking a compassionate and detail-oriented Remote Nurse Case Manager specializing in Behavioral Health to join our growing healthcare support team. In this role, you will coordinate patient-centered care plans, support individuals experiencing behavioral and mental health challenges, and collaborate with interdisciplinary healthcare professionals to improve patient outcomes.
The ideal candidate is a licensed registered nurse with strong case management experience and a passion for behavioral health. You will serve as a key liaison between patients, families, providers, and healthcare systems to ensure appropriate care, resource access, and treatment adherence. Key Responsibilities

Conduct comprehensive patient assessments related to behavioral and mental health needs.

Develop and implement individualized care management plans based on clinical evaluation.

Coordinate care between patients, physicians, therapists, social workers, and other healthcare providers.

Monitor patient progress, treatment adherence, and behavioral health outcomes.

Provide education and guidance to patients and families regarding treatment plans and available resources.

Facilitate referrals to mental health services, community resources, and specialized care providers.

Maintain accurate and confidential documentation in electronic health record (EHR) systems.

Ensure compliance with healthcare regulations, quality standards, and organizational policies.

Participate in interdisciplinary case review meetings and contribute to patient care strategies.

Support crisis intervention and escalation procedures when necessary.
Required Skills and Qualifications

Active Registered Nurse (RN) license in good standing.

Bachelor of Science in Nursing (BSN) preferred.

Strong understanding of behavioral health conditions, treatment models, and patient advocacy.

Excellent communication, care coordination, and documentation skills.

Proficiency with electronic medical records (EMR/EHR) systems.

Ability to manage multiple cases while maintaining high-quality patient care.

Experience

Minimum 3–5 years of clinical nursing experience, preferably in behavioral health, mental health, or case management.

Previous experience in care coordination, utilization management, or nurse case management is highly desirable.

Experience working with multidisciplinary healthcare teams and community resources.

Prior remote healthcare support experience is a plus.
Working Hours

Full-time remote position.

Standard schedule: Monday to Friday (8:00 AM – 5:00 PM) with occasional flexibility based on patient needs.

Some on-call or extended hours may be required depending on case demands.
