Job Summary

Mediclinic is seeking a highly skilled and experienced Senior Hospital Case Manager to join our Patient Care Coordination team. The ideal candidate will play a pivotal role in managing patient care plans, coordinating interdisciplinary healthcare services, and ensuring the delivery of exceptional, patient-centered care. This role requires strong clinical expertise, outstanding organizational skills, and the ability to navigate complex healthcare environments while maintaining compliance with regulatory standards.

Key Responsibilities

  • Lead and manage the coordination of patient care across multiple departments, ensuring seamless transitions and optimal outcomes.

  • Conduct comprehensive patient assessments, identifying healthcare needs, potential risks, and appropriate interventions.

  • Develop, implement, and monitor individualized care plans in collaboration with physicians, nurses, and allied health professionals.

  • Serve as the primary liaison between patients, families, and healthcare providers, offering guidance, education, and support.

  • Oversee case management documentation, ensuring accuracy, compliance, and timely updates in electronic health records (EHR).

  • Monitor patient progress, evaluating outcomes, and recommending adjustments to care plans as needed.

  • Facilitate communication with insurance providers, ensuring authorization and coverage for necessary treatments and services.

  • Mentor and provide guidance to junior case managers and clinical staff, fostering a collaborative team environment.

Required Skills and Qualifications

  • Registered Nurse (RN) or equivalent clinical qualification; additional certification in case management (CCM, ACM, or equivalent) preferred.

  • In-depth knowledge of hospital operations, clinical pathways, and patient care protocols.

  • Exceptional organizational and time-management skills with the ability to prioritize complex cases.

  • Excellent interpersonal, communication, and negotiation skills.

  • Strong analytical and problem-solving abilities to assess patient needs and determine appropriate interventions.

  • Proficiency with electronic health record (EHR) systems and case management software.

Experience

  • Minimum 5–7 years of clinical experience in hospital or acute care settings.

  • Proven experience in case management, patient care coordination, or care planning roles.

  • Demonstrated ability to work effectively in multidisciplinary teams and manage high patient volumes.

Working Hours

  • Full-time position.

  • Flexible schedule may be required to meet patient needs, including occasional evening or weekend coverage.

  • Strong knowledge of healthcare regulations, patient rights, and ethical standards.

  • Ability to assess, plan, implement, and evaluate patient care effectively.

  • Ability to educate patients and families on treatment plans, medication, and post-discharge care.
