Job Summary
MNC JOBS INFO is seeking a compassionate, organized, and detail-oriented Virtual Care Coordinator to join our Health Advocacy Program. This fully remote position focuses on supporting patients in navigating complex healthcare systems, coordinating care plans, and ensuring optimal health outcomes. The ideal candidate will serve as a trusted liaison between patients, healthcare providers, and internal teams, providing personalized guidance and advocacy to enhance patient satisfaction and adherence to care plans.
Key Responsibilities

Serve as the primary point of contact for assigned patients, assisting with healthcare navigation and coordination.

Schedule and manage patient appointments, follow-ups, and referrals with healthcare providers.

Educate patients on care plans, treatment options, and available health resources.

Maintain accurate and up-to-date patient records in compliance with privacy regulations (HIPAA).

Collaborate with internal teams and external providers to ensure seamless care delivery.

Required Skills and Qualifications

Experience

Working Hours

Flexible schedule may be offered, but standard hours typically align with 9:00 AM – 5:00 PM (local time).
