Job Summary:
TranscribePro USA is seeking a dedicated and compassionate Virtual Registered Nurse (RN) Case Manager to join our innovative telehealth team. Key Responsibilities:
- Conduct virtual patient assessments and evaluate healthcare needs via telehealth platforms.
- Develop, implement, and monitor individualized care plans for patients with chronic or acute conditions.
- Collaborate with physicians, specialists, and other healthcare providers to coordinate care and optimize outcomes.
- Educate patients and families on disease management, treatment plans, and preventive health practices.
- Monitor patient progress, review medical records, and adjust care plans as necessary.
- Serve as a primary point of contact for patients, providing guidance, support, and timely communication.
- Ensure compliance with regulatory, legal, and company policies related to patient care and privacy (HIPAA).
- Document all patient interactions accurately and promptly in the electronic health record system.
- Participate in quality improvement initiatives and telehealth program development.
Required Skills and Qualifications:
- Active and unrestricted Registered Nurse (RN) license in the state of practice.
- Strong clinical assessment and critical thinking skills.
- Excellent verbal and written communication skills for patient interaction and documentation.
- Proficient in telehealth platforms, electronic health records (EHR), and digital healthcare tools.
- Ability to work independently while maintaining accountability and meeting deadlines.
- Compassion, empathy, and a patient-centered approach to care.
Experience:
- Minimum of 3 years of clinical nursing experience, preferably in case management, telehealth, or chronic care management.
- Experience with insurance authorization processes, care coordination, or utilization management is a plus.
Working Hours:
