Job Summary

Progressive Technology is seeking a compassionate and experienced Telehealth Nurse to join our innovative team in providing exceptional remote patient care. As a Telehealth Nurse, you will play a pivotal role in ensuring patients receive timely and effective medical advice, monitoring, and care coordination from the comfort of their homes. Key Responsibilities

• Provide virtual health consultations, assessments, and education to patients through telecommunication platforms.
• Monitor patient health conditions, identify symptoms, and escalate cases when necessary.
• Develop, implement, and update patient care plans tailored to individual needs.
• Serve as a liaison between patients and healthcare providers to ensure seamless communication and care coordination.
• Educate patients and caregivers on disease management, medication adherence, and lifestyle modifications.
• Maintain accurate, confidential, and timely documentation of patient interactions and care plans.
• Ensure compliance with telehealth regulations, HIPAA, and other healthcare standards.
• Collaborate with interdisciplinary teams to review cases and enhance patient care outcomes.
• Act as a resource to answer patients queries and address concerns in a professional and empathetic manner.

Required Skills and Qualifications

• Active Registered Nurse (RN) license in good standing.
• Bachelor degree in Nursing (BSN) preferred but not required.
• Proven experience in clinical nursing, case management, or telehealth services.
• Excellent communication and interpersonal skills.
• Proficiency in telehealth technologies and electronic health record (EHR) systems.
• Strong critical thinking and problem-solving abilities.
• Ability to work independently and manage time effectively in a remote environment.
• Compassionate approach to patient care with attention to cultural sensitivity and individual needs.

Experience

• Minimum of 3 years of experience as a Registered Nurse.
• Prior experience in telehealth or remote patient care coordination is highly desirable.
• Experience working with chronic disease management or behavioral health is a plus.

Working Hours

• Full-time position with flexible shifts to accommodate patient needs, including evenings and weekends.
• Core hours: 8:00 AM to 6:00 PM (local time zone), with occasional on-call duties as required.
