Cigna Healthcare is seeking a compassionate and experienced Virtual Nurse Coach to join our team remotely. Key Responsibilities:

• Personalized Coaching: Provide one-on-one coaching to members via phone, video, and other digital platforms. Assist with chronic condition management, lifestyle improvements, and overall health optimization.
• Care Planning & Education: Collaborate with members to develop individualized health plans, educate them about their conditions, and provide actionable recommendations to improve their health outcomes.
• Behavioral Health Support: Offer emotional support and motivational guidance to members, helping them overcome barriers to healthier behaviors and achieve their wellness goals.
• Telehealth Integration: Utilize telehealth platforms to engage with members, coordinate care, and follow up on progress. Ensure members are using digital health tools effectively to track and manage their health.
• Health Assessments: Conduct virtual health assessments to identify health risks and provide appropriate coaching, education, and resources tailored to each member needs.
• Collaboration: Work closely with physicians, care teams, and other healthcare providers to ensure coordinated care and continuity of treatment for members.
• Data Tracking & Reporting: Track member progress, document coaching sessions, and report outcomes to leadership, providing feedback to improve care strategies.
• Support Preventive Care: Encourage members to utilize preventive services, such as screenings, vaccinations, and annual wellness visits, to support long-term health goals.

Required Qualifications:

• Active, unrestricted Registered Nurse (RN) license in the United States.
• 2+ years of experience in nursing, preferably in care management, case management, or health coaching.
• Strong knowledge of chronic disease management, wellness strategies, and preventive care practices.
• Excellent communication skills, with the ability to coach and motivate individuals remotely.
• Ability to work independently, manage time effectively, and maintain productivity in a remote work setting.
• Proficiency in using virtual care platforms and electronic health records (EHR).
• Strong problem-solving skills and the ability to make decisions based on clinical guidelines and member needs.
• Empathy and a patient-centered approach to care.

Preferred Qualifications:

• Certification in Health Coaching, Care Management, or a similar certification.
• Experience with telehealth platforms or virtual care delivery.
• Background in behavioral health or mental health coaching is a plus.
• Bilingual skills (e.g., Spanish, Mandarin) are a plus.
